Terminal lucidity has resisted systematic study less because of skepticism than because of logistics: we do not know how to bring it about. 
Families, nurses, physicians, and hospice staff describe the same sequence: a person with advanced dementia, unreachable for years, suddenly returns, speaks in whole sentences, says goodbye, and then, usually within hours or days, dies. 
Such episodes cannot be scheduled or interrupted for measurement, and are frequently over before anyone understands what is happening. 
What the field has lacked is not further cases but one that somebody brought about on purpose, and a recent report in *Frontiers in Neuroscience* may be exactly that [1]. 
## The case 
The patient was a Japanese-American woman in her 80s, 10 years into Alzheimer’s disease and five years into a hypofunction that had left her monosyllabic, incontinent, and unable to walk unaided. 
In a supervised setting, she was given five grams of psilocybin-containing mushrooms, and the acute phase was anything but gentle: profuse sweating, suspected hyperthermia, a prolonged sleep-like state. 
Yet some 19 hours later, she began to speak spontaneously and coherently about her own life, and went on for hours. 
Over the following days, the improvement continued: she recognized relatives, walked unaided, dressed herself, retrieved contextual memories, asked about people who were absent, and regained urinary continence. 
A month later, much of it still held. 
## Why this looks familiar 
Anyone who has worked with terminal lucidity will recognize the pattern, because what returns is rarely a single skill but the person as a whole—language, memory, recognition, humor, all at once. 
That simultaneity is the signature: in the largest systematic sample of contemporary cases my research group has assembled so far, 124 people, the majority with advanced dementia, roughly four out of five communicated clearly and coherently during their episode [2, 3]. 
## The differences matter more than the resemblance 
Two features set the new case apart: this woman was not dying, and her improvement lasted weeks rather than minutes or hours. 
That may look like a weakness in the analogy, but it might be the report’s most valuable feature, since it separates the return of the person from dying itself. 
This is also where the field has been moving. 
When lucidity in severe dementia was first given a formal research framework, the term was widened to paradoxical lucidity [4], and Mayo Clinic–led typology work, in which I take part, has since shown how widely they vary in duration, depth, and proximity to death, many nowhere near a deathbed [5, 6]. 
Terminal lucidity looks increasingly like a limiting case of something larger, and this report sits at the far end of that continuum, where an intervention came first. 
## The objection, and what has changed 
In the autumn of 2025, the phenomenon became a public argument in *The Wall Street Journal* between Charles Murray, Steven Pinker, and me [7, 8, 9]. 
Pinker argued that people longing for contact will read lucidity into any flicker of responsiveness, and asked for what no report had supplied—an objective indicator. 
The record answers that challenge only in part: many episodes were witnessed by clinicians and hospice staff whose accounts match the families’, while others consist of little more than a gaze, where the objection may hold. 
What the psilocybin case adds, then, is evidence that interpretation cannot manufacture: longing does not restore bladder control, and wishful thinking does not walk a bedbound woman across a room. 
Because the observations follow a dated intervention, they yield a real before-and-after, not a memory reconstructed afterward. 
## What the case does not show 
None of this makes the report strong evidence on its own, since it concerns a single patient without a control group, cognitive testing, imaging, or biomarker confirmation, and neither mixed pathology nor spontaneous fluctuation can be excluded. 
The dose was high and unstandardized, and the acute reaction was a serious safety signal in a frail patient, so nothing here establishes psilocybin as a treatment or should be read as an invitation to experiment. 
But caution is not the same thing as dismissal. 
## The hypothesis this makes testable 
For some years, one of my own working hypotheses—admittedly a highly speculative one—has been that advanced dementia may involve not only loss but blockade: memory, language, and relational knowledge are perhaps not erased in proportion to their outward disappearance, but persist latently because the damaged networks can no longer coordinate or express them, so that what is functionally inaccessible need not have been destroyed. 
Paradoxical lucidity has always been the strongest hint in that direction. 
Interestingly enough, the *Frontiers* authors propose the same logic with a mechanism attached: psilocybin loosens the rigid, over-segregated network dynamics of the diseased brain so that residual systems can briefly work together again [1], which would mean that her capacities were never rebuilt, only reopened. 
The next studies more or less design themselves, requiring standardized dosing, confirmed diagnoses, and imaging; what decides whether the two literatures belong together is whether spontaneous lucid episodes show a comparable network signature. 
## An old idea, unexpectedly useful 
The case also revives an older philosophical bridge: Bergson proposed that the brain may function not merely as the producer of cognition but as a selective, limiting organ, which Huxley later recast as a reducing valve. 
These remain speculative models and need to be treated as such [3], but speculation is not idle when it generates predictions: if damage sometimes blocks the expression of memory without extinguishing it, paradoxical lucidity and psilocybin-induced reorganization would be two doors into one room. 
## One case is one case 
A word of caution: this one may yet prove exceptional, misclassified, or impossible to reproduce, which is why it should be scrutinized hard. 
But science does not advance by refusing to look at anomalies; it advances by describing them accurately and asking what would have to be true for them to be possible. 
The lost mind may not always be destroyed, and if the door is still standing, we may soon have a clue how it opens, at least in some cases.
